EhLuh Care Solutions - Independent Social Work Practice. (travel & mileage may apply depending on location, always agreed in advance)
1. WELLBEING & SOCIAL SUPPORT SERVICE
A. Companionship Visits (non‑regulated)
Purpose: Reduce loneliness, support mental wellbeing, promote connection.
Includes:
• Friendly conversation and emotional support
• Light cognitive stimulation (memory games, music, reminiscence)
• Support with hobbies, reading, writing
• Light household companionship (not cleaning) – for people who don’t need personal care but benefit from presence and light support like sitting with the person while family steps out for example
Price: from £40/hr (travel time and mileage may apply)
Package Options:
• 4 hrs/month – £160
• 8 hrs/month – £320
• 12 hrs/month – £480
2. COMMUNITY ACCESS & LIFESTYLE SUPPORT
Purpose: Maintain independence, confidence, and engagement in the community.
Includes:
• Escort to appointments or social clubs
• Support with shopping and errands
• Attending events or faith activities
• Encouraging physical activity (walks, gentle routines)
Price: £45/hr (travel time and mileage may apply)
3. INDEPENDENT SOCIAL WORK SERVICES
For older adults, adults with disabilities, and families who need expert professional input.
A. Strength‑Based Social Work Assessments
Includes:
• Holistic needs assessment
• Recommendations
• Written report
• Certificate provider for Lasting Power of Attorney
Price: from £150/hr (depending on time/depth & travel time and mileage may apply)
B. Safeguarding Concerns & Wellbeing Checks
• Exploration of concerns
• Immediate safety planning
• Liaison with local authority or other professionals
• Written report included
Price: £95/hr (travel time and mileage may apply)
C. Mental Capacity & Decision‑Making Support
• Observation-based capacity insights
• Documentation to aid professionals/family decision-making
• Formal MCA assessments – health and welfare decisions, financial decisions, lasting power of attorney, testamentary (to make a will) and gifts (capacity to make gifts) + COP3 reports.
Price: from £350 (travel time and mileage may apply)
4. FAMILY SUPPORT SERVICES
• Family Liaison & Advocacy
• Support for families managing complex care issues
• Attendance at hospital meetings, care reviews, discharge planning
• Mediation for family disagreements around care
Price: £95/hr (travel time and mileage may apply)
• Monthly Family Update Reports
• Summary of wellbeing
• Notes from visits
• Risks or changes
• Photos if appropriate
Price: £60 per report
5. CARE NAVIGATION & COORDINATION
Purpose: Help families understand the care system without stress.
Includes:
• Guidance on funding,
• Exploration of care options (domiciliary, live‑in, residential)
• Building care plans
• Professional oversight
Price: £90/hr
One off full Care Navigation Package: £350 (includes: signposting to appropriate services, helping families understand eligibility, funding, and options, preparing them for assessments or meetings, liaising with agencies, therapists, or services, coordinating appointments, ensuring continuity and communication, and helping families stay organised)
One‑off Needs Assessment & Plan: £695
Hospital Discharge Support (2 weeks): £1,250 (includes 2 meetings + provider brokerage)
6. PREMIUM PACKAGES
A. Gold Elder Support Package – £550/month
• 4 companionship visits
• 2 community access sessions

B. Platinum Care Oversight Package – £850/month
• Weekly visits
• Professional oversight of care
